dc.description.abstractThe global health architecture is undergoing a pivotal transition driven by shifting geopolitical dynamics, declining development assistance, and an increasing demand by low- and middle-income countries for sovereign control over their national health priorities. In this commentary, we argue that the current global health system cannot respond effectively to evolving global challenges by merely preserving legacy power structures. Achieving sustainable global health security and equity requires fundamental structural reform that goes beyond institutional reorganization. We advocate for a transformed global health framework anchored in new international cooperation models, decentralized governance, and mutual accountability. Key priorities include shifting from traditional aid-dependency models toward self-sustaining national financing, addressing global structural inequities in resource allocation, and embedding local leadership into the global health research and policy agenda. Re-engineering these mechanisms is essential to building resilient health systems capable of addressing future health emergencies, climate threats, and persistent health disparities in a changing international order.en_US
